Annual Health Assessment
The Annual Health Assessment service is provide only at Puerto Rico and it translates into the following:

THE CUSTOMER provides access to insurer’s portal to MAG. These portals can be InnovaMD, Inovalon, Provinet and Humana PR.

MAG provide a web patients list on the magroupservices.com dashboard and in this list THE CUSTOMER will notify by writing the date of service next to each patient that THE CUSTOMER has had a face-to-face meeting.

MAG will transcribe the patent’s information corresponding to previous face-to-face meeting with THE CUSTOMER on the insurer's portals selected by THE CUSTOMER for the service.

If MAG needs patient information to perform the data entry, THE CUSTOMER will provide a summary in digital PDF format of the patients who do not have chronic information in the portals selected for the service.
